Core Services

Sixteen services organized around four pillars.

Practical, personal, and consistent — built to help participants assemble a life that holds together long after the program ends.

A Typical Week

What walking with us actually looks like.

A consistent weekly rhythm — small enough to be doable, structured enough to hold the week together.

  1. Mon

    Planning Session

Set the week — appointments, goals, and support touchpoints.

  2. Tue–Wed

    Mentorship & Skills

1:1 mentorship, résumé/employment work, or counseling coordination.

  3. Thu

    Accountability Call

Standing weekly check-in — wins, struggles, and adjustments.

  4. Fri

    Progress Review

Honest reflection on the week — what worked, what to change.

  5. Sun

    Community & Faith

Church, recovery community, shared meals — belonging by name.

Every plan is personalized — the rhythm flexes around work, family, court, treatment, and recovery commitments.

Common Questions

Answers to what people most often ask.

If your question isn't here, reach out — we'd rather have a real conversation than leave you guessing.

01
How long does the program last?
Our core aftercare commitment is a minimum of three months of weekly support. Many participants stay engaged longer — at a lighter cadence — as they build toward full independence.
02
Does it cost anything?
We suggest a $50/week contribution when financially feasible. First-month sponsorship is often available when donations or sponsors permit, so cost is never the reason someone can't begin.
03
Do I have to be a Christian to participate?
No. Faith-based mentorship is offered for those who want it, but no one is required to share our faith to receive support, mentorship, or practical help.
04
Is True North Transitions a treatment center or crisis service?
No. We are not a clinical treatment program or a crisis center. We provide aftercare, mentorship, and practical support — and we coordinate with licensed counselors and treatment partners. In a crisis, please call or text 988, or 911 for emergencies.
05
Can I refer a client or loved one?
Yes. Case managers and family members can start a referral anytime — we'll reach out to coordinate next steps within 1–2 business days.
06
Is support in-person or remote?
Both. Weekly accountability calls and many coaching sessions happen by phone or video, with in-person mentorship, community dinners, and connection events as participants are able.
07
What happens after the first three months?
Participants typically transition into a lighter rhythm — community connection, occasional check-ins, and ongoing access to mentorship — without ever being dropped abruptly.
