Purpose Ahead
A path toward independence and hope.
The season after treatment is the most fragile — and the least resourced.
These are the numbers we hold every day. They are the reason True North exists — not to shame anyone who has walked this road, but to change what happens next.
- ~⅔
- return to addiction within weeks of ending treatment.
- 85%
- relapse within one year of treatment.
- ~62%
- of people leaving prison are rearrested within three years.
- ~40%
- return to prison.
Aftercare — real, human, sustained aftercare — is where these numbers begin to move. See how we walk with people through it.

At True North Transitions, our mission is to help individuals and families live healthy, fulfilling lives by walking with them through aftercare — celebrating recovery, restoring purpose, and building resilient community.

Founded by two people who have walked the road — from both sides.
Tad has personally walked through addiction and sustained long-term sobriety. He leads mentorship, accountability, and the day-to-day work of helping participants rebuild.
Amanda walks alongside families touched by addiction, offering perspective, compassion, and steady support to loved ones who often feel forgotten in the recovery journey.
Together they founded True North Transitions to be the steady, practical support they wish more people had — for the person in recovery and for the family standing beside them.
More About Tad & Amanda
Endorsed by a leader in trauma, addiction, and recovery
“That is precisely the gap True North Transitions fills. Recovery does not end when treatment ends; in many ways, that is when the real work begins.”
“I offer my wholehearted endorsement and confidently recommend its services to individuals and families seeking informed, compassionate, and dependable support during the transition into long-term recovery.”
